Form Builder

The Shado FormBuilder Module enables you to create single-page online forms for your website through an intuitive and easy-to-use interface. Straker developed the FormBuilder to address client concerns over spending too much time developing online forms, many of which had a short lifespan.

This module was designed with intermediate level content managers in mind but is often used by developers and web masters as well.

By utilizing the FormBuilder it literally takes only a few minutes to create and publish a form to your site. Each form is nicely formatted according to your style sheet and the information submitted by users via the form is stored in the Shado database for easy access and review. Additionally automated email responses and on-screen messages can be linked to your forms.

Key Features

The FormBuilder contains virtually every type of field you could require for a single-page online form including:

Short and long text
Email
Check Groups and Radio Groups
Yes/No (via a radio button or check box)
List Boxes and Select Boxes
Number
Date Time
HTML

There are also a number of read-only and hidden versions of these field types.

The ability to reference any existing SOF Object is a very powerful feature and enables the FormBuilder to integrate seamlessly with existing dynamic site content.

Fields can have default values and where appropriate can be dynamic. For example a field can be designed to populate with a date X number of days from today's date, or an email field can be designed to pre-populate with a user's email address (if they are logged in to Shado, such as with an Intranet or Extranet).

Incorporate Permissions Down to Individual Forms

Each Shado User Group can be assigned permissions such as "Supervisor", or any combination of "Create", "Modify", or "Erase" in relation to the FormBuilder. These permissions can also be applied to individual forms.

Messages

The Shado FormBuilder incorporates three message types - Emails, Pop-Ups and Screen Messages. These are triggered when a user submits a form, and any given form may have any number of messages associated with it.

Results

You can filter all form submissions and download the results as a CSV file for viewing and more detailed analysis in a spreadsheet tool such as MS Excel.

Copy Existing Forms

This feature can save a lot of time when you are creating a number of forms that are similar. You simply copy the existing form, modify it and save it.

Simple Page Insertion

The FormBuilder makes it very simple to add forms onto a page. Inserting forms in the front-end is as simple as adding a new container.
